IOP/OP SUBSTANCE USE RECOVERY FEES

• Intensive Case Management

  [2x per Week and/or Weekly] ~ $150/hr

• Moderate Case Management

  (Bi-Weekly/2 per month] ~ $185/hr

• Standard Case Management [Monthly] ~ $175/hr

INSURANCE

Sway+®™ accepts PacificSource, First Choice Health, Regence and Premera BCBS insurance plans and is in the process of paneling with Moda. Sway+ also has a contract with Lyra, which is an Employee Assistance Program (EAP) provider. For all other insurances, Sway+ is an out-of-network provider (OON) and supplies statements suitable to claim out-of-network insurance benefits for submission to insurance companies for reimbursement also known as a superbill.
