IOP/OP SUBSTANCE USE RECOVERY FEES

• Intensive Case Management

  [2x per Week and/or Weekly] ~ $150/hr

• Moderate Case Management

  (Bi-Weekly/2 per month] ~ $185/hr

• Standard Case Management [Monthly] ~ $175/hr

INSURANCE

Sway®™ is an out-of-network provider (OON): I’m not on most insurance panels. I don’t accept insurance with exception of Lyra an Employee Assistance Program (EAP) provider. I provide statements suitable to claim out-of-network insurance benefits for submission to insurance companies for reimbursement.

• What is your mental health deductible?
